We are pleased that you have chosen us in taking this first step towards finding a counselor who works for you. To set up your first appointment, feel free to call our office or complete the Contact Us form on this site.

Prior to your first appointment, we will contact you and provide all of the information you need before meeting your counselor. Everything is quick, easy, secure, and confidential to complete online.

We bill most health insurance companies. Our billing experts will check your plan prior to your first session. We file claims as a courtesy to our patients; however, each patient is responsible for payment of services provided. Our billing experts will verify your insurance benefits and inform you about your specific coverage prior to your first visit.

The forms below are required prior to your coming to your first appointment. Please print out, complete, and bring the completed forms with you.
